The slope intercept form of the equation is written as:

y = mx + c

where m is the slope of the line and c is the y-intercept. 

The slope and y-intercept are given. Slope is -1 and y-intercept is 5. Using these values in the above equation, we get the slope intercept form of equation:

y = -1x + 5
y = - x + 5
