Let's take for example someone is going through a hard time and is depressed, but really doesn't seem like it. He seems to be handling life pretty well, but inside he's really struggling. Or if someone made fun of him as a joke. He can seem to be handling the joke pretty well, when in reality, he isn't. This quote is essentially saying that just because this person seems to be ok, it doesn't mean that he actually is.
