In simplest form:
2x + 5y - 35 = 0
*add 35 to both sides of the equal sign*
2x + 5y = 35

For it to be in the form of y = mx + b:
2x + 5y = 35
*subtract 2x from both sides of the equal sign*
5y = 35 - 2x
*divide by 5 on both sides of the equal sign*
y = 7 + 2/5x

I hope you got a good grade on your assignment!! ^v^
