The Modified Early Warning Score [MEWS] is part of the patient's observation chart for documenting vital signs and used to determine escalation protocol. A score is attributed to each observation based on:
a) How much urine the patient has voided
b) How many tablets you have administered
c) If you have checked the patient's temperature
d) The degree of physiological abnormality
