Pt recently felt pain at the R knee and posterior calf during exercise. She developed swelling at the calf and R ankle. Exam shows tenderness and induration at the medial head of the gastrocnemius. Moderate pitting edema and a crescent-shaped patch of ecchymosis at the medial malleolus. DX?
